I.                   Purpose

 

Tarry House Inc. assessment and referral services provide a variety of activities, including prescreening, screening, psychosocial assessment, determination of need, and referral to appropriate level of care. The provision of information on available resources is not considered a full assessment and referral program. An adequate assessment must be conducted to provide more informed referrals.

 

II.                Policy, Procedures and Definitions

 

A.     Tarry House will utilize the Ohio Department of Mental Health’s Solutions for Ohio’s Quality Improvement and Compliance (SOQIC) standardized assessment instrument in providing a comprehensive psychosocial assessments. 

1.      All personnel completing the assessment instrument will be qualified as mandated by the ODMH prior to administration of the assessment tool.

2.      The assessment process will identify appropriate levels of care for the person served and as needed, provide emergency services and crisis intervention services.

B.     The assessment service  will provide the following services in collaboration with the person served:

1.      Assessment of the needs of the person served.

2.      Identification of the choices available for community resources.

3.      Provision of informational materials pertaining to community resources, when possible.

4.      Identification of services that are culturally and age appropriate.

5.      Implementation of methods to determine if services were accessed by the persons served and to provide follow-up, when indicated.

C.     When requested, Tarry House, Inc. will provide a written summary of the assessment and referral(s) to the person served or his or her legal representation.
