I.
Purpose
The purpose of the Independent Contractors
Policy is to put into procedure, steps to ensure a system is in place to
evaluate and to retain, if appropriate, independent contractors.
II.
Policy
It is the policy of Tarry House, Inc.
to contract with individuals or entities with necessary skills sets and/or
credentials needed to provide a regular service to complete the mission of
Tarry House, Inc. Independent Contractors will be utilized when
there is no one on staff with the necessary skill sets to complete the work or
if a project is too large and/or too time consuming to be completed by Tarry
House staff. This does not include
professional repair and maintenance work, cleaning, inspections and other like
services to the facilities. A Business
Associate Agreement will be utilized when necessary to ensure contractors and
vendors are aware of and maintain privacy practices of the organization.
III.
Procedure
A. The Independent Contractor Agreement will be provided to independent contractors for review and signature for contractors needed to help Tarry House complete its mission and/or fulfill a role required by ODMH and/or CARF.
B. Independent contractors shall sign an agreement prior to services being rendered and one time annually after a decision is made by the appropriate administrative staff to retain the independent contractor. Failure to sign the Independent Contractor Agreement shall not render the individual a Tarry House employee. The administrative staff will discuss and evaluate the contractor prior to offering the independent contractor a new contract. The contractor will also be required to sign the following Tarry House documents: Code of Ethical Behavior, Conflict of Interest and Confidentiality, on an annual basis.
C. Payments to the independent contractors will occur quarterly unless different arrangements are made. Independent contractors will complete and sign the IRS 1099 form prior to receiving his/her first payment. Annually, 1099 forms will be updated as needed.
